Our social workers and nurses
are health care professionals with expertise in geriatrics,
disabilities and mental health issues, including dementia
and depression. We serve as your single point of contact,
guiding and helping you in many areas that may include, but
not limited to:
- Act as liaisons to families and as advocates for our clients
- Professional oversight and supervision of services
- Assist with problems related to the affects of dementia
and related behaviors
- Coordinate discharge planning from hospitals and rehab facilities
- Assure continuity of care and a safe transition home after
hospital stays
- Provide couseling and support for our clients and families
- Educate and provide resources for family caregivers
