At Millennia Personal Care Services, we realize
that every family situation presents new and unique challenges,
and that traditional solutions are not always best suited for
our clients. That is why we offer a FREE, NO OBLIGATION, 4
step process.
Step 1 > Initial In-Person Assessment
Our Care Manager, who is an expert in social work, mental health
and nursing, will conduct a FREE, NO OBLIGATION, in-person assessment,
who work closely with each client and loved ones in person to
carefully understand their specific condition and needs. We
can visit you in any environment, including, your home, hospital,
skilled nursing facility or rehabilitation facility.
Step 2 > Care Planning
Based on the comprehensive in-person assessment, our Care Manager
develops an individualized Care Plan and helps prioritize your
areas of concern and offer guidance on the available home care
options, community resources and other options as deemed necessary.
Step 3 > In-Person Caregiver Interview
After the in-person assessment and Care Plan has been completed,
our Care Manager will work closely with the Director of Training
& Education, and an appropriate caregiver will be matched with
our client. Our client and loved ones will have an option to
meet and interview as many caregivers as you wish to maximize
your peace of mind.
Step 4 > Ongoing Monitoring
The most important aspect of our process is the ongoing monitoring
and added care provided by our team of experts. Our Care Manager,
an expert in social work, mental health and nursing, provide
an extra pair of eyes and years of experience to monitor our
client's care and to continuously communicate with the client
and loved ones to ensure that the care plans are met. In addition,
our Director of Training & Education is responsible for training
and ongoing professional oversight of our caregivers. If at
any point you are not satisfied with our service, our Team will
promptly work with our client and loved ones to revise the plan
accordingly.
