to predict. The Backup & Respite Care Solutions is tailored
specifically to meet your last minute needs.
Registration
requirements
If you are an
employee of a sponsoring company, you must begin the registration
process before requesting any of our service. During the registration
process, you will be asked to provide detailed information in order
to assess your loved one's current situation. Please complete to
the best of your knowledge. The assessment will evaluate your loved
ones mental, physical, environmental and financial condition to
determine their ability to remain safely independent. It identifies
risks and helps determine options to reduce them. This assessment
will be stored securely in our web-based database for any future
needs. Our Quality Control Specialist nurse will schedule an initial
visit to verify your information, and will monitor your loved ones
condition every three months to keep updated information.
With advanced
preparation and monitoring, we can respond promptly to your needs
and locate an appropriate caregiver or services, should you contact
our office with your last minute needs.
