The Millennia Personal Care Services assessment is a carefully developed set of questions created by our Millennia professionals. The assessment evaluates:

• Personal Information
• Medical Information
• Activities of Daily Living
• Instrumental Activities of Daily Living
• Financial Information
• Psychosocial Information

To begin the assessment for you or your loved ones, click "Begin Assessment" and you will be asked to answer 48 questions. Once you have finished answering all the questions, click "Submit" and the assessment will be saved and evaluated by our Client Services Supervisor.

A Millennia Personal Care Services representative will contact you to arrange a second one-on-one meeting in you or your loved one's home, so that we can guide you in developing a customized plan and offer guidance on the available options.

We encourage you to take our assessment and have a clear understanding of your situation. Being involved early can bring a peace of mind of knowing you will be prepared to make the best informed decision, should any help be needed immediately.
